Webview allows patients at AMWL to view their test results, request appointments, request refills and send/receive messages without calling the office.

If you have an urgent or serious medical problem or concern, please go to an emergency room or call us at 801-272-4111 for advice.

You can not use Webview unless you have already received a user name and password. To receive them, complete the following steps:

1.   Read the AMWL Internet Policies Document    MS Word 2003   PDF

2.   Print, fill out, sign the AMWL Webview Policy Agreement     MS Word 2003   PDF

3.   Fax or mail the Agreement to the number or location listed in the Contact Us tab.

4.   Call 801-272-4111 during office hours to receive your user name and password.

Please be aware of the following before using Webview:

1.   The use of Webview is a conditional privilege that can be revoked at any time and without notice.

2.   The parts of my record at AMWL that are visible via Webview are only a portion of my medical record. A complete copy is available through the AMWL medical records department.

3.   My test results will be visible in Webview as soon as they are placed in my chart at AMWL and before my provider has time to interpret them. I am aware that there is a 5-business-day turn around time for my provider to interpret my test results, although significantly abnormal test results are handled on a more urgent basis, as appropriate.

4.   I understand that there is a 2-business-day turn around time for my messages, appointment requests and prescription refills to be answered.

5.   I understand that sending more than 1 message requesting the same action from any provider or department at AMWL within a 2-business-day period will result in termination of Webview privileges.

6.   I understand that Webview does not allow individual communication with providers and employees at AMWL.
